J gastroenterol: the effect of gastric cancer screening program on the long-term prognosis of patients with gastric cancer

Gastric cancer is one of the most common malignant tumors in the world. According to the statistics of GLOBOCAN 2020, there were nearly 1.1 million cases of gastric cancer worldwide in 2020, ranking the third among all kinds of cancer-related deaths. There are obvious regional differences in the incidence rate and mortality of gastric cancer, about 75% of which occur in Asia.

The Korean national cancer screening program (kncsp) provides upper gastrointestinal examination (UGIS) or upper gastrointestinal endoscopy for adults over 40 every two years. The purpose of this study was to evaluate the effect of cancer screening program on the long-term prognosis of patients with gastric cancer.
The researchers constructed a nationwide population cohort based on three national databases, and included 46701 patients with gastric cancer diagnosed between 2008 and 2009. They were followed up to the end of 2019. The primary end point was long-term survival.

After a median follow-up of 10.5 years, 18614 patients (39.9%) died. The survival rate in the screening group was higher than that in the non screening group (65.8% vs 49.1%). The risk of death from gastric cancer was reduced by 53% in the screening group (HR 0.47).

The risk ratio for gastric cancer specific mortality was lower in patients undergoing upper gastrointestinal endoscopy than in patients undergoing UGIS screening (0.36 vs 0.69). The risk of death from gastric cancer was reduced by 35% in patients who had been screened within two years before the diagnosis of cancer, but only by 19% in patients who had been screened more than three years since the last screening.
In conclusion, this study emphasizes the positive role of gastric cancer screening in the survival and prognosis of patients with gastric cancer. Patients with upper gastrointestinal endoscopy or gastric cancer screening within two years before diagnosis had the best prognosis.
