Nodule "calcification", is it canceration? Experts remind: 4 methods to judge the risk of malignant change

Abstract:Everyday,manypatientsleavemessagesasking:"Whatshouldwedoifwefindthyroidnodules?DoesB-ultrasoundshownodulecalcificationmeancanceration?"Today,withthesequestions,WeiBangge,DirectorofThyroidDepartmentofK

Every day, many patients leave messages asking: "What should we do if we find thyroid nodules? Does B-ultrasound show nodule calcification mean canceration?"

Today, with these questions, Wei Bangge, Director of Thyroid Department of Kunming Zhongyan, will give you the science popularization!

1. What should I do if I find thyroid nodules?

Director Wei Bangge said that most of the thyroid nodules were benign, while 85% of the malignant nodules were papillary carcinoma. The prognosis was good after timely treatment. However, a few undifferentiated cancers and medullary cancers pose a great threat to life.

Therefore, if the nodule is found, first go to a professional and regular hospital for further examination and evaluation by a professional doctor to determine whether the thyroid nodule is benign or malignant, and then determine the treatment plan.

2. What is the difference between low echo and high echo? How likely is malignancy?

Different tissues have different densities, and the attenuation of sound waves is different when they pass through.

The normal thyroid tissue is generally "moderate echo", while the diseased part is generally hyperechoic, hypoechoic, and anechoic. The higher the echo, the greater the tissue density, and the lower the echo, the higher the nodule malignant rate.

3. Is there a high probability of canceration of a single nodule? Must multiple nodules be benign?

In general, a single "hypoechoic solid nodule" is more likely to become cancerous than "multiple nodules". Especially for young male friends, if it is a single solid nodule, it is necessary to check and rule out the possibility of canceration, but it is not absolute. For women over 40 years old, multiple thyroid nodules may also be malignant.

If you want to judge whether the thyroid nodule is cancerous, you need to judge it by comprehensive pathological examination such as B ultrasound and puncture.

4. B ultrasound shows nodule calcification, is it canceration?

Ultrasonography is one of the important examinations to judge the benign and malignant nodules. Calcification is an important reference index, but it is not the only index. Look at the description of the index, especially the gravel like small calcification, living cluster calcification or visible strong light spots;

Ultrasonographic Manifestations of Thyroid Carcinoma

If the nodule is accompanied by large calcification, the probability of malignancy is relatively reduced, and about 20% of them may be thyroid cancer, of which follicular adenocarcinoma accounts for a large proportion;

Calcification is an important reference index to judge the canceration of the nodule. At the same time, it needs to be combined with whether the border of the nodule is clear and the echo of the nodule.

Therefore, thyroid nodule calcification and thyroid cancer have a certain relationship!

4 methods

Know whether the nodule will deteriorate

Most thyroid nodules are benign, while malignant nodules account for a small part.

Benign, has little impact on the body; Malignant influence is greater, even endangering life.

1. Medical history

The following five are high risk factors, which should be noted:

Whether they have received a large amount of ionizing radiation, such as nuclear power plant leakage, head and neck radiotherapy, etc;

Men and children;

Hoarseness, dyspnea, dysphagia and other symptoms;

Nodules grow rapidly in short term;

History of thyroid cancer, or thyroid cancer in a family member

2. Check calcitonin

Blood test is required. If calcitonin is greater than 100pg/mL, it indicates that medullary thyroid carcinoma is likely.

3. Check neck color ultrasound

If the following words are included in the report, it indicates that the possibility of thyroid cancer is high:

Solid hypoechoic nodules;

Abundant internal blood flow;

Irregular shape, unclear edge and absence of halo;

Micro calcification, needle like diffuse distribution or cluster distribution;

Abnormal cervical lymph nodes.

4. Fine needle biopsy

Puncture is the most direct way to judge benign and malignant. If the result is malignant, thyroid cancer can be basically diagnosed.

Last reminder:

You don't need to scare yourself when you find a thyroid nodule; It is also important to evaluate whether the nodules are benign or malignant.

Most benign nodules and small cancers less than 1 cm can be treated by microwave ablation, without damaging the gland or sequelae; If it is confirmed that the nodule is malignant after puncture, it needs to be resected in time to avoid further metastasis.
